
We're not connected to PacWest but have seen very similar things with Verizon in the past. It was usually that they turned up a new gateway and started sending calls from it. We would not have a route back to then so they would overflow to another gateway (the second call) but never CANCEL'd the first, so you now have two distinct calls but only one (generally the second call) with audio. I'll be curious to see if it is something similar.
