
Seems to be that TekStar is a carrier in that NPA/NXX and they managed to figure out a way to charge outrageous carrier access charges to support among other things a free conference calling service (most likely it's the other way around with what is supporting what). Because of this Level3 and Bandwidth are both not reliably completing calls there. In the case of Bandwidth they're just refusing the calls all together, while Level 3 (at least on the Vector platform) will let you complete a call there every once in a while.
From a ticket with Level 3 earlier in the week:
Notes : We have seen unprecedented growth of traffic to these very rural areas. There is currently no ETA for a fix. Level3 has a limited amount of capacity in these particular areas and at this time we will not be augmenting. This increase in traffic is the result of questionable activities on the part of several independent telecom providers that result in extremely high access fees for carriers such as Level3 and we will continue to protect ourselves and our customers from this type of activities and the cost that goes along with it.
