From what little I've heard about it today, seems to be Antigua and Barbados that have mentioned the most. I don't have any other data, unfortunately.

It's called Wangiri (Japanese for "one and cut") since it started in Japan years ago. If I understand things correctly, that's not actually cramming; the telcos are playing games with the settlement fees somehow.
